Our survey is now closed.

On Tuesday 22nd November between 10am and 12pm, we'll be carrying out an Enter and View visit to Graham Road Surgery and Horizon Health Centre.

We'll be speaking to patients and staff to find out how they feel about the care their receive from their surgery. If you aren't available to speak to us on the day, you can share your feedback by filling in a short survey. The survey is open until 8th December.

All local Healthwatch have a legal power to visit health and social care services and see them in action. The purpose of these Enter and View visits is to collect evidence of what works well and what could be improved to make people’s experiences better. An Enter and View visit isn't an inspection. During a visit we'll speak to people who are available on the day. This may include patients, staff, and family members.

After our visit, we'll write up a report that we can share with the service provider. Our report will include the feedback people have shared with us, any evidence we've recorded, and our overall insights. All feedback is anonymised, so people who speak to us during our Enter and View visits cannot be identified.
