GP surgeries and Primary Care Networks

Primary care networks (PCNs) are made up of multiple GP surgeries that work together in a specific area. The groups of surgeries will also work with local community, mental health, social care, pharmacy, hospital, and voluntary services.

PCNs usually cover between 30,000 and 50,000 patients.

You can download a tube-style map below, which will tell you which PCN your GP surgery is part of. The name of the PCN is the text in bold at the end of each tube line. For example, Gloucester Road Medical Centre and Horfield Health Centre are part of the Phoenix PCN.

If you would like the map sent to you as an image file or PDF, please email helen@healthwatchbnssg.co.uk

Integrated Care Partnerships

Each PCN falls under an integrated care partnership (ICP), and these are also shown on the map. ICPs are groups of NHS providers that work together to deliver care. In the Bristol, North Somerset, and South Gloucestershire area, there are six ICPs, which are also shown on the map:

  • Woodspring (North Somerset)
  • Weston, Worle & Villages (North Somerset)
  • North and West Bristol (Bristol)
  • Inner City and East Bristol (Bristol)
  • South Bristol (Bristol)
  • South Gloucestershire (South Gloucestershire).

Integrated Care Systems

Integrated care systems (ICSs) oversee health and wellbeing services. There is an ICS in every part of England - the ICS in our local area covers Bristol, North Somerset, and South Gloucestershire (BNSSG). The King's Fund have created a useful video, explaining this new way the NHS is structured. Click to watch.

If you would like to find out more about the ICS in our area, you can download a copy of our poster below. If you require this poster in an alternative format, please email helen@healthwatchbnssg.co.uk

Louise Ansari, National Director at Healthwatch England, on the Health and Care Bill recieving Royal Assent from Her Majesty The Queen:

"Today marks an important milestone in creating an NHS that makes it easier for everyone to get the care they need.

Integrated care is an ideal incentive for health and social care services, councils and the voluntary sector to work together to design services that work better and reflect the way people use them. The new structures will work best if people, and in particular those from seldom heard groups, get truly involved and have a say in planning our health and care. By integrating services across communities, we have a chance to address delays and gaps in care.

As a statutory champion for patients’ rights, we look forward to playing our part, supporting the NHS to hear and act on the issues that people face so that professionals and the public can work together to overcome the current challenges and build a better NHS for generations to come."
