New mentoring scheme for Diabetes patients in Nailsea

The community hub in Nailsea, Number 65, has set up a new project to help people with diabetes stay active and well.

Diabetes patients can improve their health by adjusting their lifestyle habits and this new scheme involves using activity trackers help people to monitor their progress.

A mentor meets with patients fortnightly to talk through their data and discuss how they can stay fit, well and active. Staff at number 65 said it is this personal touch – meeting people face-to-face – which will make a difference to the project rather than just handing out the monitors and leaving people to get on with it.

Generally the mentoring lasts 12 weeks but the plan is to continue helping people if they want it. 

Patients can sign up to the scheme themselves or they can be referred by their doctor.
